Symptoms and conditions assessed

This includes abdominal pain, diarrhoea, constipation, bloating, altered bowel habit, gastrointestinal bleeding, iron-deficiency anaemia, coeliac disease, peptic ulcer disease, unexplained weight loss and other digestive symptoms.

A structured assessment

The first step is a careful history. Tests are then selected according to the clinical question and may include blood or stool tests, imaging, gastroscopy, colonoscopy, capsule endoscopy or other specialised investigations.

Persistent diarrhoea

The causes are varied and include infection, inflammation, medication effects, malabsorption and functional bowel disorders. Investigation is guided by duration, associated symptoms and warning features.

Constipation and altered bowel habit

Many cases can be managed conservatively, but a new or persistent change in bowel habit—particularly when accompanied by bleeding, anaemia, weight loss or a relevant family history—may require further investigation.

Small-bowel assessment

Video capsule endoscopy can be useful in selected patients when the small bowel needs to be examined beyond the reach of routine gastroscopy and colonoscopy. The recorded study is then carefully reviewed and interpreted.

Digestive symptoms can be difficult to describe and can significantly affect everyday life. The consultation is designed to give patients time to explain what they are experiencing, followed by a clear discussion of what may be happening and what the next steps should be.
